Job summary

Providence at Home with Compassus in Seattle is seeking a compassionate Hospice Registered Nurse (RN) to deliver person-centered care in patients' homes. You will assess needs, develop care plans with the IDG, and provide professional nursing care across the nursing process.

The role emphasizes end-of-life comfort, collaboration, and documentation, with on-call duties and adherence to standards. A valid RN license and BLS within 30 days are required; one year hospice/home health experience

Qualifications

Associate or Bachelor’s degree from an accredited School of Nursing.
1 year of hospice or home health RN experience preferred.
Current RN license in State of Employment.
BLS certification within 30 days of hire.

Responsibilities

Completes assessment of patients and families to determine Hospice needs.
Writes and initiates the Plan of Care in collaboration with IDG.
Regularly reevaluates patient, family, and caregiver needs and revises the Plan of Care.
Initiates the Plan of Care and makes necessary revisions as patient status and needs change.
Develops a care plan in conjunction with the patient, family, caregiver, and IDG.
May administer medications and treatments as prescribed by the IDG-approved Plan of Care.
Participates in on-call duties as defined by the on-call procedures.
Maintains productivity standards as set by organizational guidelines.
