Job summary

Affinity Health Management in Ohio seeks a Clinical Manager (RN) to ensure coordinated patient care and to supervise clinical personnel in hospice/home care services.

The role requires strong nursing leadership, adherence to standards, and collaboration with medical directors to optimize outcomes. Prior hospice/home care and supervisory experience are preferred; active Ohio RN licensure is required. This is a full-time position with opportunities for quality improvement and team mentoring.

Qualifications

• RN license to practice in the state of Ohio.
• BSN preferred; ADN with 1 year nursing experience acceptable.
• Hospice/home care experience with supervisory responsibilities.
• Minimum 2 years of management or supervisory experience in a clinical setting.
• CPR certification; excellent observation and communication skills.

Responsibilities

• Receives case referrals and assigns hospice personnel to cases.
• Reviews and evaluates cases to guide clinicians and ensure quality home care.
• Reviews medical diagnoses, prognosis, medications and clinical course.
• Manages medical records and EMR workflow; supports plan of care.
• Assists in setting immediate and long-term therapeutic goals with the team.
• Attends case conferences to coordinate care and interdisciplinary input.
• Conducts quarterly record reviews and communicates findings to leadership.
• Assists in interviewing and orientation of new employees.
• Supports policy development and ongoing education programs.
• Ensures adherence to professional standards and promotes customer service.
• Participates in community health activities and quality improvement projects.
• Performs other duties as delegated by Clinical Director/Director of Patient Care Services.
