Job summary

Kindful Health seeks a Hospice Sales Liaison in Plano, TX to establish and grow hospice referrals, build relationships with physicians, hospitals, discharge planners and families, and execute a regional sales/marketing plan.

The ideal candidate has a Bachelor’s degree (preferred) and 2+ years of hospice sales experience, with strong communication and CRM skills to support admissions and care coordination in the local market.

Qualifications

• Bachelor's degree preferred and hospice sales experience.
• Experience building relationships with physicians, hospitals, discharge planners, and case managers.
• Proficient in using CRM systems to track referrals and opportunities.

Responsibilities

• Generates qualified referrals to achieve hospice admissions.
• Maintains up-to-date records of referral sources in the CRM.
• Assists care center in obtaining documentation for referral-to-admission.
• Evaluates referrals for admission suitability and provides counseling to referrers and families.
• Coordinates with care team to ensure resources meet patient needs.
