Kindful Health is seeking a Hospice Sales Liaison in the Bedford/Ft Worth, TX area to establish new business contacts, maintain relationships, and implement a sales plan for the assigned region.

The role requires strong communication with patients, families, physicians, social workers, discharge planners, and vendors. A bachelor’s degree is preferred, with at least two years Hospice sales experience.

Qualifications

• 2+ years Hospice sales experience preferred.
• Bachelor's degree is preferred.
• Strong communication and relationship-building abilities.

Responsibilities

• Generates qualified referrals to achieve hospice admissions.
• Maintains up-to-date referral source records in CRM.
• Assists in obtaining documentation for referral-to-admission.
• Evaluates referrals for appropriate hospice admission.
• Provides counseling to referrers, patients and families about care options.
• Coordinates with care team to allocate resources and services.
• Acts as liaison during transition from discharge to hospice services.
• Develops and maintains client relationships within a competitive market.
