Job summary

Madrona Hospice Care Inc. in Concord, NH is seeking a Hospice Marketer / Community Liaison to develop referral relationships and generate hospice referrals through compliant business development.

The role acts as a liaison with hospitals, SNFs, physician offices, and other partners, coordinating referrals with intake and clinical teams. It emphasizes ethical marketing, territory management, and community education.

Qualifications

• Bachelor’s degree in healthcare, business, marketing, communications, or related field preferred.
• Previous hospice, home health, post-acute, hospital, SNF, healthcare sales, or business-development experience preferred.
• Existing relationships with healthcare referral sources within the assigned territory are highly desirable.
• Knowledge of hospice services and the Medicare hospice benefit preferred.

Responsibilities

• Develop new hospice referral sources within the assigned territory and strengthen existing partnerships.
• Maintain an organized territory plan and prioritize high-potential accounts.
• Maintain consistent field activity (7–10 meaningful contacts per business day) including facility visits, physician offices, and meetings with care teams.
• Coordinate referrals with intake and clinical teams and communicate discharge plans as needed.
• Ensure compliant, ethical marketing and avoid inducements; protect patient information.
• Collaborate with Intake, Administrator, Nursing/Clinical Director, physicians, social workers, and billing/quality staff.
• Maintain accurate marketing records and reporting (daily activity logs, pipeline, and outcomes).
• Provide educational presentations on hospice philosophy, eligibility, and benefits.
