Job summary

Hireology is seeking a Hospice Marketer / Community Liaison for a hospice client to grow referrals and partnerships across the community. The role emphasizes relationship-building with hospitals, physicians, SNFs and other healthcare providers, plus field outreach to educate and convert referrals.

Qualifications

• Minimum 2+ years in hospice, home health, healthcare, or senior-care marketing.
• Established healthcare referral network preferred.
• Strong knowledge of hospice services and Medicare hospice eligibility.
• Excellent communication, presentation, and relationship-building skills.
• Willing to spend significant time in the field and travel.
• Ability to manage territory autonomously and meet targets.

Responsibilities

• Develop and maintain referral relationships with hospitals, physicians, SNFs, assisted living, home health agencies, rehab facilities, senior communities, case managers and social workers.
• Generate qualified hospice referrals through consistent business development and community outreach.
• Conduct in-person sales calls, presentations, facility visits, networking and educational meetings.
• Educate referral partners about hospice services, eligibility criteria, benefits, and referral process.
• Identify new referral opportunities and strategies to increase census and market penetration.
• Maintain regular communication and follow-up with existing and prospective referral sources.
• Build relationships with hospital discharge planners, case managers, social workers, physicians, admins and directors of nursing.
• Coordinate with admissions and clinical teams to ensure smooth referral and admission.
• Track sales activities, referrals, conversions, and follow-ups in CRM.
• Participate in networking events, healthcare associations and community events.
• Monitor market activity and identify opportunities to strengthen competitive position.
• Meet weekly and monthly activity, referral, admission, and business-development goals.
• Represent the hospice organization professionally and ethically.
