Job summary

Hireology is seeking a Hospice Marketer / Community Liaison to grow referrals and partnerships within the Texas community. You will build relationships across hospitals, SNFs, physician groups, and home health agencies, while educating partners on hospice services and eligibility.

The role requires strong networking, field outreach, and a history of successful referral generation. You will collaborate with admissions and clinical teams to ensure smooth referrals and admissions, with regular CRM

Qualifications

• 2+ years in hospice, home health, healthcare, or senior-care marketing/business development preferred.
• Prior hospice marketing experience and established healthcare referral network are strongly preferred.
• Demonstrated history of generating healthcare referrals and developing professional referral relationships.
• Knowledge of hospice services, Medicare hospice eligibility, and the referral process preferred.
• Strong sales, presentation, networking, communication, and relationship-building skills.

Responsibilities

• Develop and maintain referral relationships with hospitals, physicians, SNFs, assisted living communities, home health agencies, rehabilitation facilities, senior communities, case managers, social workers, and other healthcare professionals.
• Generate qualified hospice referrals through consistent business development and community outreach.
• Conduct in-person sales calls, presentations, facility visits, networking activities, and educational meetings.
• Educate referral partners about hospice services, eligibility criteria, benefits, and the referral process.
• Identify new referral opportunities and develop strategies to increase census and market penetration.
• Maintain regular communication and follow-up with existing and prospective referral sources.
• Build relationships with hospital discharge planners, case managers, social workers, physicians, administrators, and directors of nursing.
• Coordinate closely with admissions and clinical teams to help ensure a smooth referral and admission process.
• Track sales activities, referrals, conversions, referral sources, and follow-up activities using the company's CRM or reporting system.
• Participate in networking events, healthcare associations, community events, facility functions, and other business-development activities.
• Monitor market activity and identify opportunities to strengthen the organization's competitive position.
• Meet established weekly and monthly activity, referral, admission, and business-development goals.
• Represent the hospice organization professionally and ethically throughout the community.
