Job summary

Trustbridge Hospice, part of Empath Health, seeks an organized Care Team Assistant in West Palm Beach. This onsite, full-time role runs Monday–Friday, 3:00 p.m.–11:30 p.m., supporting the front desk and ensuring a compassionate experience for patients and families during life’s transitions.

You will handle clerical duties, chart accuracy, and team coordination, while collaborating with leadership and other staff. Strong communication and MS Office skills are essential.

Qualifications

• High school diploma or GED required; two years of admin experience in hospice or related field preferred.
• Strong verbal and written communication skills with solid customer service experience.
• Proficient in basic office software and able to learn additional systems as needed.

Responsibilities

• Perform clerical duties for the team, including filing, processing, scanning, mail handling, and updating timesheets and volunteer logs.
• Maintain accuracy of patient charts and distribute paperwork to disciplines and partners in a timely manner.
• Support weekly IDT meetings and team events; track census changes and updates in electronic systems.
• Assist with patient equipment requests and coordinate DME removal as needed.
