Job summary

Empath Health's Trustbridge Hospice in West Palm Beach is seeking an organized and friendly Care Team Assistant to support the front desk and ensure a compassionate experience for patients and families during one of life’s meaningful transitions. The role is onsite, full-time, with Monday–Friday, 3:00 p.m.–11:30 p.m.

Qualifications

• HS diploma or GED; 2 years admin experience in hospice or related field preferred.
• Excellent verbal and written communication skills; solid customer service.
• Excellent documentation and organizational skills, including time management, multi-tasking and prioritizing.
• Demonstrated ability to learn MS Word, Excel, Outlook and additional software as needed.

Responsibilities

• Perform clerical duties for the team such as filing, processing/entering and copying documents (including timesheets for HHA’s, volunteer hours in Raiser's Edge, etc.); distributing mail, faxes, and phone messages in a timely manner; maintain office and team supplies.
• Ensure information on patient charts is accurate. Distribute the appropriate paperwork to the different disciplines, departments and/or community partners in a timely fashion.
• Collaborate with Team Leadership to inform and prepare for weekly IDT meetings, supports, events, agency meetings and educational opportunities; participate as assigned.
• Assist in tracking team census by monitoring status changes, levels of care, admissions, deaths, discharges, and transfers via electronic software; update patient care board and calculate census timely.
• Work with team members to cover patient care needs; handle phone requests calmly and professionally per agency communications.

Trustbridge Hospice, a part of Empath Health, is seeking an organized and friendly Care Team Assistant in our team in West Palm Beach.

This position is onsite. This entry level, full-time position works Monday through Friday, 3:00 p.m – 11:30 p.m., supporting the front desk and ensuring a seamless, empathetic experience for patients and families during one of life’s most meaningful transitions.

Since 1978, Trustbridge Hospice has cared for more than 200,000 South Florida families. As a community-based nonprofit, we provide 24/7 hospice and palliative care, along with caregiver support and bereavement services for families facing serious illness.

What You’ll Do

• Perform administrative clerical duties for team such as filing, processing/entering and copying documents (including timesheets for HHA’s, volunteer hours in Raiser’s Edge, etc.); distributing mail, faxes, and phone messages in a timely manner; maintain office and team supplies.

• Communicating patient equipment needs to DME and maintaining records of transactions and coordinating DME removal as identified.

What You'll Need

• Education and/or Experience: High school diploma or general education degree (GED); minimum 2 years administrative experience in hospice or related field preferred.
• Excellent verbal and written communication skills. Solid customer service skills.
• Excellent documentation and organizational skills, including time management, multi-tasking and prioritizing.
• Exhibit self-direction, independent judgment, discretion and ability to work and collaborate with all team members, including volunteers.
• Strong computer knowledge in MS Word, Excel and Outlook, and the capability to learn additional software as needed.
• Demonstrated ability to be sensitive to the needs of the staff and Empath Health clientele as well as flexibility within an organized system.
• Medical terminology background preferred.
