Job summary

Trustbridge Hospice, part of Empath Health, seeks an organized and friendly Care Team Assistant in West Palm Beach to support the front desk and ensure a compassionate experience for patients and families.

This onsite, entry-level, full-time role runs Monday through Friday 8:00 a.m.–4:30 p.m., performing clerical duties, chart accuracy, and coordination with team members to keep operations running smoothly.

Qualifications

• High school diploma or GED required or equivalent experience.
• Minimum 2 years administrative experience in hospice or related field preferred.
• Strong verbal and written communication skills.

Responsibilities

• Perform administrative clerical duties: filing, processing/entering documents, distributing mail and messages.
• Ensure patient charts information is accurate and distribute paperwork to disciplines and partners.
• Support team in weekly meetings and events; assist in census tracking and status changes.
