Job summary

Trustbridge Hospice, part of Empath Health, seeks an organized Care Team Assistant in Boca Raton to support the front desk and patient families. This entry-level, full-time role is onsite with weekday hours, 8:00 a.m.

to 4:30 p.m., Monday through Friday, focusing on administrative tasks that keep the team responsive and compassionate during transitions. You will handle filing, chart updates, scheduling support, and coordination with DME and other departments.

Qualifications

• High school diploma or GED completed.
• Minimum 2 years administrative experience in hospice or related field preferred.
• Excellent verbal and written communication skills.
• Strong documentation and organizational skills; ability to multitask.
• Proficiency in MS Word, Excel and Outlook; ability to learn other software.

Responsibilities

• Perform administrative clerical duties: filing, processing/entering and copying documents, distributing mail and messages, and maintaining supplies.
• Ensure accurate patient chart information and distribute paperwork to disciplines and partners promptly.
• Coordinate weekly IDT meetings, events, and educational opportunities with team leadership.
• Track team census by monitoring status changes, admissions, discharges, and transfers; update care boards.
• Support coverage of patient care needs and handle phone requests professionally.
• Communicate patient equipment needs to DME and maintain transaction records.
