Job summary

Hospice of Florida seeks a self-driven Community Liaison to promote our hospice services across St. Johns & Clay Counties. You’ll cultivate trust-based relationships with referral sources, physicians, and healthcare partners to drive census growth and awareness of our compassionate care.

This role emphasizes building meaningful connections, educating families about hospice services, and representing our mission with empathy across the community.

Qualifications

• Bachelor’s degree in related field preferred; experience may substitute.
• Minimum two years of sales/marketing in healthcare required; hospice/palliative marketing preferred.
• Skilled at establishing relationships with key staff and contracts.
• Proven ability to develop and maintain accounts with physicians and healthcare professionals.

Responsibilities

• Establish and nurture relationships with physicians and healthcare partners.
• Serve as advocate for hospice services through regular outreach.
• Grow presence by promoting hospice as provider of choice in the service area.
• Drive referral growth via visits with referring physicians and care partners.
• Support patients and families by guiding initial conversations and obtaining consent.
