Job summary

Holisticare Hospice is seeking a self-driven Community Liaison to serve the Southern Ocean County area. You will build trust-based relationships with referral sources, physicians, and healthcare partners to increase awareness and access to hospice services.

Responsibilities include growing census through outreach, visiting physicians, and presenting compassion-driven care. The role requires healthcare marketing experience, strong communication, and reliable transportation; a Bachelor’s degree is

Qualifications

• Bachelor’s degree preferred; other experience may substitute.
• Minimum two years of sales/marketing experience in healthcare required; hospice/palliative marketing preferred.
• Proven ability to establish/maintain relationships with key staff and accounts.
• Willingness to drive with reliable transportation and valid license.

Responsibilities

• Establish and nurture relationships with physicians and healthcare partners to grow census.
• Advocate for hospice services through regular outreach and education.
• Grow presence by promoting hospice services across the service area.
• Drive referral growth through visits with referring physicians, discharge planners, SNFs, social workers, and other professionals.
• Lead initial conversations about hospice care and obtain informed consent with empathy.
