Job summary

Chapters Health System is seeking an Account Development Liaison in Lakeland, FL, to educate community care partners about hospice and palliative care services and eligibility. The role also promotes Home Health services to non-hospice patients within the territory.

The candidate will drive referral generation by building relationships with hospitals, physicians, SNFs, and home health agencies, traveling across the designated service area and maintaining robust CRM records.

Qualifications

• Bachelor's degree in related field preferred or equivalent combination of education and experience.
• Minimum of two (2) years of professional medical sales experience preferred (other sales experience may be considered).
• Bi-lingual in Spanish preferred.
• Proficiency with Outlook, Word, Excel, and PowerPoint.

Responsibilities

• Cultivate and maintain quality professional relationships with hospitals, physicians, SNFs, and home health agencies within the territory.
• Provide ongoing education and data updates to referral sources regarding patient care pathways and program updates.
• Develop and execute data-driven quarterly sales plans focusing on admission goals and program growth.
• Maintain territory accounts with up-to-date profile information and timely CRM documentation.
