Job summary

The Care Team Hospice is seeking a driven Community Care Liaison to grow our presence in Grand Rapids and surrounding communities. You will build partnerships with physicians, hospitals, and referral sources to increase awareness and enrollment of hospice services.

As the community face of our hospice, you will educate providers, patients, and families about the value of compassionate, patient-centered care, travel within the territory, and collaborate with the team to achieve census targets.

Qualifications

• 1-2 years healthcare outside sales experience.
• 1-2 years Hospice or Home Health sales experience required.
• Excellent communication and networking skills.

Responsibilities

• Build and expand relationships with physicians, healthcare partners, and key decision-makers to drive referral volume and market growth.
• Educate the community and share the value of hospice-centered care as an advocate for hospice services.
• Achieve census and admission targets by positioning hospice as a preferred provider in the community.
• Engage regularly with healthcare providers to create strong, collaborative partnerships focused on patient well-being.
• Lead hospice discussions with patients and families, communicating services with professionalism and empathy.
