Job summary

The Care Team Hospice is seeking a driven Community Care Liaison to grow our presence in the community. You will build relationships with physicians, hospitals, and other referral sources to increase awareness and utilization of our hospice services in the Cleveland/Highland Heights area.

This high-impact role focuses on census growth through partnerships, educating providers and families about the value of comfort-focused care, and guiding patients toward compassionate, high-quality end-of-life

Qualifications

• Bachelor’s degree or equivalent required.
• 1–2 years healthcare outside sales experience; hospice/home health sales preferred.
• Willingness to travel regularly within assigned geography and for training.
• Strong ability to educate providers and families about hospice value.

Responsibilities

• Build relationships with physicians, healthcare partners, and decision-makers to drive referral volume.
• Educate community on hospice value and patient-centered care.
• Exceed census and admission targets by positioning hospice as preferred provider.
• Engage with providers and referral sources to create collaborative partnerships.
• Lead hospice discussions with patients and families, communicating services with empathy.
