Job summary

Alternate Solutions Health Network seeks a Community Educator to inform healthcare providers about our hospice/home health services in Bloomington and surrounding areas. You will professionally represent the company to gain and maintain community accounts and support patient transitions across care settings.

Responsibilities include building clinician relationships, educating on home healthcare benefits, and coordinating referrals with internal teams.

Responsibilities

• Educate healthcare professionals about company services, e.g., physician offices, long-term care facilities, rehabs and hospitals.
• Develop and maintain relationships with healthcare professionals in surrounding communities.
• Create community awareness of home healthcare needs and benefits.
• Network and represent the Company at business organizations.
• Develop and organize community education programs for healthcare workers.
• Maintain existing accounts and educate clients for services.
• Coordinate referral admissions to ease transitions with internal staff.
• Follow up with referrals via phone, letters, and visits.
• Prepare weekly marketing reports for supervisor and monthly expense reports.
• Set weekly, monthly, and yearly goals for new accounts and referrals.
• Attend in-service trainings and mandatory agency meetings.
• Maintain a professional appearance as a company representative.
• Attend ongoing seminars to stay updated on Medicare/Medicaid home healthcare policies.
