Job summary

Alternate Solutions Health Network, LLC seeks a Community Market Manager and Educator to drive growth across assigned agencies and manage referral relationships. You will partner with clinical teams to align patient care with market strategies, identify future patients, and ensure compliant, efficient home care services.

The role requires strong communication, travel within the region, and a passion for improving patient outcomes in post-acute care settings.

Qualifications

3–5 years in market growth in home care or medical field.
RN/LPN/PT/PTA/LSW preferred in the service state.
Strong communication with patients, families, physicians and co-workers.
Ability to travel daily within assigned region.
Valid driver's license and auto insurance.
Detail-oriented with critical thinking.

Responsibilities

Direct community growth and strategic plan for assigned agencies and accounts.
Collaborate with region leadership to implement growth strategies and prioritize key customers.
Coordinate with clinical team on complex patient cases for best outcomes.
Identify future patients and determine home care eligibility.
Review insurances and medical documentation for home care suitability.
Coordinate health care services as ordered by physicians.
Assist discharge planning with hospital/facility staff.
Maintain relationships with patients, facilities, physicians, and team members.
Develop and execute market-specific annual plans and quarterly updates.
Manage time/resources and territorial call coverage.
