Job summary

Enhabit Home Health & Hospice in Denton, TX, seeks an experienced nursing leader to oversee hospice services, ensure care coordination and adherence to standards, and support clinicians and patients through every stage of care.

The role emphasizes collaboration with physicians, referral sources, and families, with a focus on compliance, documentation, and delivering compassionate, high-quality home-based care.

Qualifications

• Must be a graduate of an approved school of professional nursing.
• Must maintain a valid nursing license per state requirements.
• Minimum of two years of nursing experience.
• Minimum of one year of hospice or home health experience.
• Knowledge of federal, state and local laws governing home care.
• CPR certification required.
• Valid state driver license and automobile liability insurance.
• Basic technology skills including use of mobile devices.

Responsibilities

• Coordinate hospice services per standards and procedures.
• Review patient information at starts of care, recertifications, resumption of care or evaluations.
• Facilitate relationships among physicians, referral sources, patients, caregivers and staff.
• Schedule patients to field clinicians and maintain effective care coordination.
