Enhabit Home Health & Hospice seeks a nurse leader to ensure integrated home health services, coordinating care across physicians, patients, and caregivers. You will review start of care and recertifications, and foster collaboration among staff to maintain high-quality patient outcomes.

Ideal candidates hold an approved nursing degree and active license, with CPR certification and a valid driver license. On-site attendance is essential to supervise, collaborate, and drive care excellence.

Qualifications

• Graduate of an approved school of professional nursing.
• Must maintain a valid clinical license per state requirements.
• CPR certification as required by position.

Responsibilities

• Coordinate overall home health services per standards and procedures.
• Review and approve start of care, recertification, resumption of care, or evaluation visits.
• Facilitate relationships between physicians, referral sources, patients, caregivers, and staff.
• Schedule patients to field clinicians and communicate with all stakeholders to maintain care continuity.
