Job summary

Enhabit Home Health & Hospice in Texas seeks an experienced RN leader to oversee patient care coordination across the home health and hospice continuum. You will ensure start-of-care processes, collaborate with physicians and caregivers, and supervise scheduling and communication among field clinicians to maintain optimal continuity of care.

The role emphasizes compliance with federal, state and local regulations and requires strong communication skills and a commitment to delivering

Qualifications

• Must be graduate of an approved school of professional nursing.
• Must maintain a valid RN license per state requirements.
• Minimum two years nursing experience; minimum one year hospice/home health.
• Familiar with federal, state and local laws for home care operations.
• CPR certification required; valid state driver license and auto liability insurance.
• Basic tech skills including Microsoft Office and mobile devices.
• Preferred management experience and experience with Homecare Homebase (HCHB).
• Bilingual English/Spanish preferred for clear communication with patients and caregivers.
• In-office attendance is essential for supervision, teamwork and task completion.

Responsibilities

• Coordinate hospice services to patients according to standards and procedures.
• Review and approve patient information at start of care, recertification, resumption or evaluation.
• Facilitate relationships between physicians, referral sources, patients, caregivers and staff.
• Schedule patients to field clinicians and maintain communication to ensure care continuity.
