Enhabit Home Health & Hospice seeks an experienced nursing leader to oversee coordination of home health services, ensuring adherence to standards and regulatory guidelines.

You will review patient information for start of care and care continuity, and facilitate collaboration among physicians, caregivers, and staff. In-office attendance is required to drive supervision and service quality.

Competenze

• Graduate of an approved school of professional nursing.
• Valid clinical nursing license and adherence to state board rules.
• Two years nursing experience within the last 36 months.
• One year home health experience.
• Knowledge of federal, state and local laws governing home care operations.
• CPR certification.
• Valid state driver's license and automobile liability insurance.
• Basic technology skills including mobile device usage.
• Preferred experience in a management role.
• Preferred Homecare Homebase (HCHB) experience.
• Preferred bilingual English/Spanish communication.

Mansioni

• Coordinate home health services delivered to the patient per standards of practice and company procedures.
• Review and approve patient information at start of care, recertification, resumption of care, or evaluation visit.
• Facilitate relationships between physicians, referral sources, patients, caregivers, and employees.
• Schedule patients to field clinicians and ensure communication with field staff, patients, physicians, referral sources, caregivers, and other providers to maintain care coordination.
