Job summary

Enhabit Home Health & Hospice is seeking a registered nurse to oversee care coordination across hospice services, ensuring compliance with standards and company procedures. The role interfaces with physicians, patients, caregivers, and field staff to maintain seamless care delivery.

Ideal candidates have an RN credential, strong communication skills, and the ability to manage scheduling and regulatory requirements.

Qualifications

• Graduate of an approved school of professional nursing and licensed to practice in the state.
• Must have CPR certification and valid state driver's license.
• Minimum two years nursing experience with hospice or home health.
• Experience with regulatory guidelines governing home care.
• Preferred management experience and bilingual English/Spanish.

Responsibilities

• Coordinate hospice services ensuring standards of practice and procedures.
• Review and approve patient information at start of care, recertification, or evaluation.
• Facilitate relationships between physicians, patients, and caregivers.
• Schedule patients to field clinicians and maintain care continuity.
