Job summary

Enhabit Home Health & Hospice is seeking a skilled Registered Nurse leader to oversee home health coordination and ensure care aligns with standards and regulations. You will collaborate with physicians, caregivers and staff to maintain seamless patient flow and scheduling across care teams.

The role requires an active RN license, minimum two years of nursing experience (home health preferred), CPR, and a valid driver license. Excellent communication and tech skills are essential for success.

Qualifications

• Must be a graduate of an approved school of professional nursing.
• Active and valid nursing license per state requirements.
• Minimum two years of nursing experience within the last 36 months.
• At least one year of home health experience.
• Knowledge of federal/state/local home care laws.
• CPR certification required.
• Valid driver license and automobile insurance.
• Ability to use a mobile device and basic tech.
• Experience in management and Homecare Homebase preferred.
• Bilingual English/Spanish preferred.

Responsibilities

• Coordinate overall coordination of home health services to meet standards and procedures.
• Review and approve patient information during start of care, recertification, resumption of care, or evaluation visits.
• Facilitate communication between physicians, referral sources, patients, caregivers and staff.
• Ensure scheduling of patients to field clinicians and maintain care coordination.
