Job summary

Enhabit Home Health & Hospice is seeking a skilled registered nurse in a leadership role to coordinate hospice services and ensure high-quality patient care in a home health setting. The position emphasizes collaboration with physicians, referral sources, patients and caregivers to maintain seamless care coordination.

The role requires a current RN license, minimum two years nursing experience including hospice/home health, and proficiency with applicable laws.

Qualifications

• Graduate of an approved school of professional nursing.
• Valid state nursing license (or compact license per state).
• Minimum two years nursing experience.
• Minimum one year in hospice or home health.
• Knowledge of federal/state/local laws for home care operations.
• CPR certification.
• Driver license in current state of residence.
• Automobile liability insurance as required by law.
• Basic technology skills including smartphone usage.
• Preferred: experience in a management role.
• Preferred: experience with Homecare Homebase (HCHB).
• Preferred: bilingual (English/Spanish).

Responsibilities

• Coordinate hospice services to meet standards of practice and company procedures.
• Review and approve patient information during start of care, recertification, resumption of care or evaluation visit.
• Facilitate relationships between physicians, referral sources, patients, caregivers and staff.
• Schedule patients to field clinicians and maintain communication with all stakeholders to ensure continuity of care.
