Job summary

Enhabit Home Health & Hospice is seeking a dedicated registered nurse to oversee coordination of home health services and ensure high‑quality patient care. The role emphasizes collaboration with physicians, patients, and caregivers to maintain seamless care delivery.

Ideal candidates will have an active nursing license, 2+ years of recent nursing experience (including home health), CPR certification, and strong communication skills.

Qualifications

• Graduate of an approved school of professional nursing.
• Must maintain a valid clinical license as required by state rules or compact license.
• Minimum two years nursing experience within the last 36 months and at least one year in home health.
• CPR certification required and valid state driver license with automobile liability insurance.

Responsibilities

• Coordinate home health services to ensure care meets standards and company procedures.
• Review and approve patient information at start of care, recertification, resumption of care, or evaluation visits.
• Facilitate communication among physicians, referral sources, patients, caregivers and staff to maintain care coordination.
• Schedule patients with field clinicians and ensure effective communication across all parties.
