Job summary

Qualifications

• Licensed practical or professional nurse with RN license in good standing.
• Excellent observation, verbal and written communication skills required.
• Prolonged standing and walking; ability to lift and transfer patients.

Responsibilities

• Completes and maintains comprehensive patient/family assessments to identify hospice needs.
• Develops, implements and revises plan of care with patient/family input.
• Administers medications and treatments as prescribed; documents outcomes.
• Counsels patient and family/caregiver; provides health education.
• Acts as Case Manager and coordinates care with the interdisciplinary team.
• Participates in on-call duties and ensures needed equipment and services are available.
• Supervises ancillary personnel and supports ongoing professional development.
• Maintains safety and quality through continuous improvement activities.
