Affinity Hospice Management LLC is seeking a dedicated Registered Nurse to lead in-home hospice care. The RN will assess patient and family needs, develop and adjust care plans, and coordinate with physicians to ensure quality end-of-life care.

The role emphasizes community health education, direct patient care, and collaboration with an interdisciplinary team to support patients in their homes and communities.

Qualifications

• Graduate of an accredited school of nursing.
• Licensed as a Registered Nurse in Virginia.
• Excellent observation, verbal and written communication skills, problem solving skills, and basic math abilities.

Responsabilités

• Conduct initial and ongoing comprehensive assessments of patient and family to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Create and revise plans of care in collaboration with the patient, family, and interdisciplinary team.
