Job summary

Affinity Hospice Management LLC is seeking a Registered Nurse to plan, organize, and direct hospice services in patients' homes within the Portland, OR area. You will build on community resources to meet individual and family needs in home settings.

You will assess patients, develop care plans, administer prescribed medications, educate families, and coordinate interdisciplinary care. Strong communication and organizational skills are essential for success.

Qualifications

• Graduate of an accredited school of nursing.
• RN license in good standing to practice.
• Minimum of 1 year professional nursing experience within the last 3 years, or BSN with 2 years’ experience in public health/home care/hospice preferred.

Responsibilities

• Complete initial and ongoing assessments of patient and family to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise the care plan with patient/family involvement.
• Administer medications and treatments as prescribed.
• Counsel patients and families on nursing and related needs.
• Assist patients with activities of daily living and promote self-sufficiency and comfort.
• Coordinate patient care as Case Manager when assigned.
