Qualifications

• Graduate of an accredited school of nursing.
• Licensed RN in good standing to practice in Virginia.

Responsibilities

• Performs initial and ongoing comprehensive patient and family assessments to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Develops and updates plan of care with patient/family involvement.
• Administers medications and treatments as prescribed.
• Counsels patient and family on nursing needs and self-care.
• Maintains and submits accurate clinical notes and reports changes in condition.
• Coordinates with physicians and the hospice interdisciplinary team.
