Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse licensed in good standing to practice.
• Minimum of 1 year of professional nursing experience in the last 3 years; or a BSN with at least 2 years’ experience in public health, home care, or hospice is preferred.
• Management experience not required; responsible for supervising hospice aides.
• Offer contingent upon references and background checks.
• Strong observation, communication, problem solving, and basic math skills.
• Able to lift, stand, and assist patients; capable of physical tasks and driving as needed.
• CPR certification required (hands-on verification preferred).

Responsibilities

• Complete initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs.
• Provide nursing care using the nursing process and develop a care plan with patient/family involvement.
• Initiate and revise the plan of care as patient needs change and coordinate patient care as Case Manager when assigned.
• Communicate with physicians and care team; document clinical notes and changes.
• Educate patients and families; provide care instructions and health guidance.
• Maintain a safe environment and ensure continuity of care across settings.
• Participate in on-call duties and supervise ancillary personnel.
• Pursue ongoing professional development and continuing education.
