Qualifications

• Graduate of an accredited school of nursing.
• Licensed RN in good standing to practice.
• Experience in home care, hospice, or public health preferred.
• Valid driver’s license with reliable vehicle.
• CPR certification required.
• Strong communication and documentation skills.

Responsibilities

• Assess patient and family to determine hospice needs and plan care.
• Provide professional nursing care using the nursing process.
• Develop and revise care plans with patient and family.
• Administer medications and treatments as prescribed by the physician.
• Coordinate patient care as Case Manager when assigned.
• Complete accurate clinical notes and communicate with physicians.
• Participate in on-call duties as defined by policy.
• Supervise hospice aides and ensure equipment arrangements.
• Promote safety and continuity of care with the interdisciplinary team.
