Job summary

Enhabit Home Health & Hospice is seeking a dedicated clinician to represent the branch in external transitional care activities and educate patients, families, and partners about Enhabit services. You will integrate guidelines and coordinate patient-centered transition plans to promote quality and efficiency.

The role requires a clinical education graduate, state licensure, and at least one year in home care, with solid knowledge of regulatory guidelines and basic mobile tech skills.

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience in home care.
• Must have demonstrated experience and understand federal, state, and local laws and regulatory guidelines governing the operations of a hospice.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Represent the branch in external transitional care activities, and in professional contacts with patients, families, physicians, hospitals, facilities, senior living communities, professional associations, and similar health groups and institutions, to apprise them of the availability of Enhabit Home Health & Hospice services.
• Integrate clinical guidelines, protocols, and other metrics to develop patient-centered transition plans that promote quality and efficiency.
• Serve as a public awareness representative and will be responsible for education related to provided services.
