Job summary

Enhabit Home Health & Hospice is seeking a qualified professional to support branch operations, coordinate transitional care for patients, families, physicians, and hospitals, and promote the availability of Enhabit services.

The role requires graduation from an approved school of clinical education, state licensure, at least one year in home care, and familiarity with federal/state regulations. A valid driver's license and reliable transportation are also required.

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience in home care.
• Must have demonstrated understanding of federal, state, and local laws and regulatory guidelines governing hospice operations.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Represent the branch in external transitional care activities with patients, families, physicians, hospitals, facilities, and professional groups to promote Enhabit Home Health & Hospice services.
• Develop patient-centered transition plans by integrating clinical guidelines and metrics for quality and efficiency.
• Serve as a public awareness representative and educate about the provided services.
