Job summary

Enhabit Home Health & Hospice is seeking a dedicated healthcare professional to represent the branch in external transitional care activities and in professional contacts with patients, families and facilities. This role emphasizes educating communities about Enhabit's home health and hospice services.

You will develop patient-centered transition plans using clinical guidelines to improve quality and efficiency, in support of compassionate, high-quality care in the home setting.

Qualifications

• Graduate of an approved school of clinical education.
• Licensed to practice in the state where you work.
• At least one year of experience in home care.
• Knowledge of federal, state, and local hospice laws and regulations.
• Basic tech skills including operation of a mobile device.

Responsibilities

• Represent the branch in external transitional care activities and professional contacts to promote Enhabit Home Health & Hospice services.
• Develop patient-centered transition plans using clinical guidelines to promote quality and efficiency.
• Serve as a public awareness representative and educate about provided services.
