Job summary

Enhabit Home Health & Hospice is seeking a dedicated professional to support the branch in coordinating transitional care and engaging with patients, families, and health partners. The role emphasizes adherence to clinical guidelines and delivery of high-quality, patient-centered care across home health services.

The ideal candidate will have a clinical education background, state licensure, and home care experience, plus strong regulatory knowledge and technology skills to support service

Qualifications

• Graduate of an approved school of clinical education.
• Licensed in the state where practicing.
• At least one year of experience in home care.
• Knowledge of federal, state, and local health laws and guidelines governing hospice operations.
• Basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Represent the branch in external transitional care activities and professional contacts with patients, families, physicians, hospitals, facilities, and health groups to inform them of Enhabit services.
• Integrate clinical guidelines and protocols to develop patient-centered transition plans that promote quality and efficiency.
• Serve as a public awareness representative and provide education related to the services offered.
