Job summary

Enhabit Home Health & Hospice is seeking a clinician to support transitional care across patients, families, physicians, and facilities. You will integrate clinical guidelines and tools to develop patient-centered transition plans that improve quality and efficiency of home-based care.

Responsibilities include representing the branch in public and professional engagements, educating payors and partners about hospice services, and ensuring compliance with applicable laws while delivering

Qualifications

• Graduate from an approved school of nursing, therapy, or social work.
• Must be licensed in the state of practice.
• Minimum two years clinical experience.

Responsibilities

• Represent the branch in transitional care activities with patients, families, physicians and facilities.
• Develop patient-centered transition plans to promote quality and efficiency of home care.
• Serve as a public awareness representative and educate payors about hospice services.
