Job summary

Enhabit Home Health & Hospice is seeking a team member to support external transition activities, promoting our home-based care services. This role focuses on coordinating patient-centered transition plans by applying clinical guidelines and metrics to improve quality of care.

The position also includes representing Enhabit to the public and educating stakeholders about our services, while ensuring compliance with applicable laws and regulations.

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience in home care.
• Must understand federal, state, and local laws and regulatory guidelines governing hospice operations.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Represent the branch in external transitional care activities and professional contacts to apprise them of Enhabit Home Health & Hospice services.
• Integrate clinical guidelines, protocols, and other metrics to develop patient-centered transition plans that promote quality and efficiency.
• Serve as a public awareness representative and provide education related to provided services.
