Qualifications

• Graduate of an approved school of clinical education.
• Licensed to practice in the state where you work.
• At least 1 year of experience in home care.
• Knowledge of federal, state, and local laws governing hospice operations.
• Basic technology skills, including operation of a mobile device.

Responsibilities

• Represent the branch in external transitional care activities with patients, families, physicians, hospitals, facilities, and other health partners.
• Develop patient-centered transition plans aligned with guidelines and metrics to promote quality and efficiency.
• Serve as a public awareness representative and educate about provided hospice services.
