Enhabit Home Health & Hospice is seeking a qualified professional to support branch operations in transitional care, coordinating with patients, families and providers to ensure seamless service. Ideal candidates bring a clinical education background, valid licensure in the relevant state, and at least one year in home care, with comfort using mobile technology.

Qualifications

• Graduate of an approved school of clinical education.
• Licensed in the state where they practice.
• At least 1 year experience in home care.
• Knowledge of federal/state/local hospice regulations.
• Basic technology skills, including mobile devices.

Responsibilities

• Represent the branch in external transitional care activities and professional contacts with patients, families, physicians, hospitals, facilities, and health groups.
• Develop patient-centered transition plans that promote quality and efficiency by integrating clinical guidelines and protocols.
• Serve as a public awareness representative and educate communities about provided services.
