Qualifications

• Must be licensed in the state where they currently practice.
• Must have at least one year experience in home care.
• Must have demonstrated knowledge of federal, state, and local laws and regulatory guidelines governing hospice operations.
• Must have basic technology skills, including operation of a mobile device.

Responsibilities

• Represent the branch in external transitional care activities and inform stakeholders of Enhabit Home Health & Hospice services.
• Develop patient-centered transition plans using clinical guidelines, protocols, and metrics.
• Serve as a public awareness representative and educate about provided services.
