Job summary

VitalCaring is seeking a Care Transition Coordinator to connect patients, families, and partners with our hospice services. This field-based role emphasizes relationship-building, efficient transitions, and growth within a mission-driven organization.

You will work with referral sources, clinical teams, and community partners to ensure timely admissions and high-quality patient access, with travel across the designated territory.

Qualifications

• Bachelor's degree or equivalent experience in healthcare, business, marketing, or related field.
• Minimum of two years in healthcare or related sales.
• Strong communication, relationship-building, and influencing skills.
• Valid driver's license and reliable transportation.

Responsibilities

• Serve as the primary liaison between referral sources, patients, families, and clinical teams
• Execute structured sales calls, build relationships, and drive referral growth
• Collaborate with hospitals, SNFs, physicians, and community partners
• Coordinate care transitions and ensure timely admissions to home health/hospice
• Document activities in CRM and EMR systems
