Job summary

St. Croix Hospice seeks a Care Transition Coordinator/Account Executive - Hospitals to build strong referral relationships with hospitals and designated communities in the Pewaukee area. The role educates providers and families about hospice care and supports intake to ensure timely referrals.

You will coordinate marketing and sales activities, monitor referral sources, and collaborate with clinical partners to drive patient referrals while helping the marketing team meet annual volume targets.

Qualifications

• Associate’s degree preferred in marketing, business administration, healthcare, or related field.
• 3+ years of proven success in a sales role, preferably within hospice, medical device, or pharmaceutical sales.
• Ability to position services to and deal tactfully with customers and clients.
• Demonstrates strong communication, negotiation, and public relations skills.
• Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
• Ability to pass DHS background study.

Responsibilities

• Meet admission and call volume targets.
• Coordinate daily sales and marketing operations including marketing initiatives.
• Build and monitor customer perceptions of St. Croix Hospice as a high-quality provider.
• Collaborate with clinical partners to drive patient referrals and business growth.
• Document sales activity in the CRM per requirements.
• Support budgeted sales volumes and monitor cost effectiveness of marketing efforts.
• Assist with coordinating and scheduling marketing events and vendor fairs.
• Mentor and support the Care Transition Team and orient new staff in marketing.
