Job summary

St. Croix Hospice is seeking a Care Transition Coordinator/Account Executive - Hospitals in Oak Creek, WI.

This role focuses on developing strong relationships with hospital referral sources and educating teams about hospice services to improve timely referrals. Responsibilities include coordinating marketing efforts, maintaining CRM records, and collaborating with clinical partners to drive growth, while supporting the Care Transition Team and regional leadership.

Qualifications

• 3+ years of proven success in a sales role, preferably within hospice, medical device, or pharmaceutical sales.
• Associate's degree preferred in marketing, business administration or healthcare.
• Ability to position services to customers and clients with tact.

Responsibilities

• Develop and maintain positive relationships with referral sources and designated communities.
• Provide education on hospice care to referrals and patients.
• Assist intake to ensure timely and accurate referral coordination.
• Coordinate daily sales and marketing operations including marketing initiative implementation.
• Document all sales activity in the CRM as required.
• Collaborate with clinical partners to drive patient referrals and growth.
• Support marketing events, vendor fairs, and exhibits across territories.
