Job summary

St. Croix Hospice is seeking a Care Transition Coordinator/Account Executive to build strong hospital partnerships and drive patient referrals. You will educate hospital staff and families about hospice care, coordinate referral workflows, and support intake to ensure timely admissions.

You will manage marketing initiatives, monitor market trends, and collaborate with clinical teams to align services with community needs, all while maintaining CRM records and hitting sales targets in Wisconsin.

Qualifications

• Associate's degree in marketing, business, healthcare, or related field.
• 3+ years of proven sales experience, preferably in hospice or healthcare.
• Ability to establish relationships with hospitals and referral sources.

Responsibilities

• Meet admission and call volume targets.
• Coordinate daily sales and marketing operations and events.
• Build and monitor perceptions of St. Croix Hospice as a high-quality provider.
• Collaborate with clinical partners to drive patient referrals.
• Document sales activity in the CRM per organizational requirements.
• Educate patients, families, and caregivers on hospice and informed decision making.
• Assist leadership with budget and marketing planning.
