Qualifications

• Understanding of hospice philosophy and death/dying issues.
• CPR certification required.
• Must be a licensed driver with an insured vehicle.

Responsibilities

• Assess psychosocial status of patients and families/caregivers related to terminal illness and environment; communicate findings to the team.
• Carry out social evaluations including family dynamics and caregiver abilities; provide crisis intervention when necessary.
• Educate interdisciplinary group on cultural needs of patients and families; support advanced directives.
• Participate in development of individualized plan of care with patient and family; attend interdisciplinary meetings.
• Assist physician and team in understanding social and emotional factors related to health problems and death/dying issues.
• Maintain accurate clinical notes, coordinate care, and ensure continuity of care.
