Job summary

Qualifications

• MSW degree from CSWE-accredited program.
• Minimum of one year healthcare/hospice experience preferred.
• Hospice care experience preferred.
• Strong verbal and written communication and organizational skills.
• Contingent upon satisfactory references and criminal background checks.
• Prolonged walking or standing; good vision and hearing.
• Must be a licensed driver with insured vehicle.
• CPR certification required (hands-on verification).
• Covid-19 vaccination required.

Responsibilities

• Assess psychosocial status of patients and families and relay findings to the care team.
• Conduct social evaluations including family dynamics and plan interventions; provide crisis support.
• Educate on culture, traditions, and advanced directives.
• Contribute to the plan of care and attend interdisciplinary meetings.
• Assist in understanding social/emotional factors related to health and end-of-life issues.
• Assist with funeral planning, financial, legal, and healthcare decisions.
