Job summary

Qualifications

• MSW from CSWE-accredited program.

Responsibilities

• Assesses psychosocial status of patients and families and communicates findings to the care team.
• Carries out social evaluations and counsels patient and family; provides crisis intervention when needed.
• Educates on advanced directives to patients and families.
• Participates in development of the individualized plan of care with the patient and family.
• Assists with understanding social and emotional factors related to health problems and end-of-life care.
• Assists with funeral, financial, legal, and healthcare decision planning as needed.
• Represents the social work perspective in interdisciplinary group meetings and collaborations.

Education

MSW (Master of Social Work) degree from CSWE-accredited program
