Job summary

Affinity Hospice Management LLC in Ohio seeks a Medical Social Worker (MSW) to implement standards of care for hospice patients and families. As a core member of the Interdisciplinary Group, you will provide psychosocial support based on assessments and care plans.

You will perform psychosocial assessments, education, care planning, crisis intervention, and coordination with medical staff, community resources, and families to ensure compassionate end-of-life care.

Qualifications

• MSW from an accredited school (CSWE preferred).
• Minimum 1 year healthcare/hospice experience preferred.
• Strong verbal and written communication skills.
• Licensed driver with reliable vehicle.
• CPR certification required (hands-on).
• Covid-19 vaccination required.

Responsibilities

• Assess psychosocial status of patients and families; communicate findings to the care team.
• Conduct social evaluations and plan interventions based on assessment findings.
• Educate interdisciplinary team on cultural needs and family dynamics.
• Educate patients and families on advance directives and care options.
• Develop individualized care plans with patient and family input.
• Assist team in understanding psychosocial factors related to health and death.
• Assist with funeral planning, finances, and healthcare decision responsibilities.
• Complete and maintain accurate clinical notes and documentation.
• Coordinate with physicians; obtain orders as required.
• Provide information and referrals to patients and caregivers.
• Maintain liaison with community agencies and resources.
• Develop contracts with public/private agencies as resources for patients.
• Report mandated disclosures per regulations and protect patient safety.
• Report suspected abuse/neglect to appropriate authorities.
• Participate in interdisciplinary group meetings and collaborate with team.
• Participate in on-call duties; support RN Case Manager with logistics.
• Pursue ongoing professional development and performance improvement.
• Fulfill assigned case tasks and other duties as delegated.
