Job summary

Affinity Hospice Management LLC in Akron, OH is seeking a Medical Social Worker (MSW) to implement standards of care and provide psychosocial support to patients and families within the hospice program.

You will assess psychosocial status, participate in interdisciplinary meetings, educate families on advance directives, and coordinate care with the team to address social and emotional needs. A MSW degree and licensure are required.

Qualifications

• Requires a Master’s degree in Social Work (MSW) from an accredited program.
• Minimum of one year experience in health care; hospice experience preferred.
• Demonstrates strong verbal and written communication and organization skills.
• Must provide satisfactory references and pass criminal background checks; CPR certification and Covid-19 vaccination required.

Responsibilities

• Assess the psychosocial status of patients and families/caregivers and communicate findings to the care team.
• Conduct social evaluations addressing family dynamics, caregiver abilities, and coping strategies; provide crisis intervention when needed.
• Educate the care team on cultural needs and advance directive preparations.
• Participate in the development of individualized care plans with patient/family input.
• Coordinate with physicians and interdisciplinary team to address social and emotional factors.
• Assist with funeral planning, financial, legal, and health care decision responsibilities.
• Attend interdisciplinary group meetings and maintain accurate clinical notes.
